Third Sector Integrated Care Fund 2020 – Sept 2022

Please be advised that a decision has been taken to extend the deadline for applications to the Integrated Care Fund from 6th to the 23rd of December.

Following this the appraisal and decision making process will follow as scheduled in the guidance.

The Third Sector Integrated Care Fund originally issued during 2016 is now seeking to attract applications from Third Sector providers for the period April 2020 - end March 2022.

The Health and Social Care Partnership are asking for Third Sector Organisations to consider applications toward the following areas;

  1. Projects where the main focus is on reducing isolation.
  2. Re-settling services, Home readiness services, Minor adaptations.
  3. Projects where the main focus is on targeting and reducing health inequalities through early intervention and prevention.
  4. Collaborative delivery of Social Prescribing activities
  5. Building the capability and quality of Social Enterprise in supporting Health and Social Care.

Collaborative applications are encouraged within these areas of delivery to maximise reach and impact.

Applications will be considered to a maximum of £100,000 per year, larger fully collaborative bids may be considered by the Grant Recommendation and Monitoring Panel.

The process of application will involve a review of the submitted application by a

‘Grant Recommendation and Monitoring Panel’ (The Panel) and approval by the South Lanarkshire Strategic Planning Group. VASLan, as the Third Sector Interface, will administer the process of application and monitoring of approved grants.

New project applications will be given priority consideration.  Providers who are currently in receipt of ICF funding and who wish to extend or continue projects similar in design, scope and reach will be required to provide a detailed evaluative report relating to the existing project together with a plan outlining the project’s future impacts.

The Panel may consider bids outside of the identified priority areas providing the applicant can clearly show the need for the provision in South Lanarkshire and can demonstrate a strong impact on one or more of the 9 National Health and Wellbeing Indicators.

Timescales for submission of an application are as follows: -

Completed application together with any supporting information by no later than Monday 23rd December 2019.

The Panel will assess the submissions and make recommendations to the Strategic Planning Group with successful applicants being informed by mid-February 2020.

We would ask that any initial questions regarding the process or to register your interest by means of completing the attached application and submitting the completed document to Sandra Renicks
