Delayed Discharge: A Conversation with the Third Sector

South Lanarkshire Health and Social Care Partnership has made big strides in reducing the length of time people are delayed in hospital but we want to improve this even further.

There is a big role for the third sector in supporting the delayed discharge / admission avoidance pathways. This could be getting shopping, switching on heating, small repairs, preventing loneliness for example. Small things that get in the way of people getting home.

We would like a conversation on how we can progress this work and align it to the improving delayed discharge work that has already had success. VASlan, along with representatives of the Health and Social Care Partnership will be hosting a virtual conversation with members of the sector on Friday 28th May from 10am - noon to discuss the issues around delayed discharge and how we may work together to reduce it even further.
